It is with heavy hearts that we announce the closure of Sparlin Mental Health, effective November 19, 2021. Our practice provided professional development, high-quality training and support to more than 1,000 community health workers, mental health practitioners and human service leaders. And we are proud to have employed some of the most talented clinical professionals around — showing up each day exemplifying devotion, compassion and commitment to quality. We are honored to have our legacy live on through every one of them.

Notes For Patients
The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than seven years after the last date of services you received from this Practice, although most records will be held for at least 10 years from the date of last service, depending upon applicable state and federal record retention requirements.
